MediShield Life, MediSave And MediFund: What Families Need To Know

5 min read
MediShield Life, MediSave And MediFund: What Families Need To Know

Singaporeans enjoy good and affordable healthcare with heavy government subsidies, Medisave, MediShield and Medifund. Purchasing MediShield early helps to manage your healthcare costs and gives you peace of mind. MediShield is an affordable, basic medical insurance scheme

Most families only find out how this works when they’re standing at a hospital counter. Give it twenty minutes beforehand, and the parts that count for a family are fewer than the acronyms suggest.

How the layers work, in order:

  • MediSave: your own savings, in your own CPF account, for approved medical uses
  • MediShield Life: national health insurance covering large hospital bills, for everyone
  • MediFund: a safety net for those who still can’t pay after the first two

They aren’t alternatives. A bill typically draws on subsidies first, then insurance, then MediSave, then whatever is left.

MediShield Life

  • Covers all Singapore Citizens and Permanent Residents, for life.
  • Covers pre-existing conditions: nobody is excluded.
  • Newborns are covered automatically from birth, including for congenital and neonatal conditions, with no application needed. For a new parent, nothing else on this page is more important.
  • Designed around subsidised wards: Class B2 and C in public hospitals. It pays out at those levels, so a private hospital bill leaves a much larger gap.
  • Premiums are payable from MediSave, and rise with age.
  • You still pay part. There’s a deductible you meet first and a co-insurance portion after that. MediShield Life is designed for large bills, not routine ones.

Integrated Shield Plans

Private insurance layered on top, for those who want private hospitals or higher ward classes.

  • The MediShield Life portion continues underneath: an Integrated Shield Plan wraps around it instead of replacing it.
  • MediSave can be used for part of the premium, up to a withdrawal limit. The rest is cash, and for higher-tier plans that cash portion becomes substantial.
  • Premiums rise steeply with age. Model this before committing: a plan that’s affordable at 35 can be difficult at 65, and downgrading later isn’t always straightforward.
  • Riders reduce the co-payment. Since regulatory changes, riders must leave some co-payment in place instead of covering everything.
  • Buy for the baby early if you want one, before any condition appears that could be excluded by the private insurer. MediShield Life itself covers pre-existing conditions; the private layer may not.

What this means for a family

  • Pregnancy and delivery: MediSave can be used for the delivery plus an additional amount for pre-delivery expenses such as antenatal visits, scans and tests, which can be claimed retrospectively. Keep every receipt from the first appointment, because this is routinely missed.
  • Either parent’s MediSave can be used.
  • MediShield Life covers pregnancy complications, though not a normal delivery.
  • The newborn is insured from the moment of birth, which is the safety net if anything goes wrong.
  • Childhood vaccinations on the national schedule can be paid from MediSave at approved clinics, and they’re commonly paid in cash by people who didn’t know.
  • Orthodontic treatment isn’t covered by MediSave, which surprises many families budgeting for braces.

Things that get confused

  • CareShield Life is a different scheme, for long-term care if you become severely disabled. Not the same as MediShield Life.
  • The Child Development Account is a savings scheme with government matching, not healthcare.
  • Subsidies come first. Ward-class subsidies in public hospitals reduce the bill before any insurance applies, so ward class makes a big difference to the final figure.

What to do

  1. Check what Integrated Shield Plan you’ve, if any. Many people are unsure, and it determines what a hospital admission costs.
  2. Decide about a plan for the baby in the first months.
  3. Keep antenatal receipts for the pre-delivery MediSave claim.
  4. Ask the hospital’s billing office before a planned admission for an estimate showing subsidies, insurance and your expected out-of-pocket portion.
  5. Ask about the medical social worker if a bill is unaffordable. That’s what MediFund exists for and there’s no cost to asking.

Limits, premiums and thresholds change. Check current figures on the CPF and Ministry of Health websites instead of relying on any article, including this one.

Frequently asked questions

What is MediShield Life?

Basic national health insurance covering all Singapore Citizens and Permanent Residents for life, including those with pre-existing conditions. It’s designed for large hospital bills at subsidised ward levels, with premiums payable from MediSave.

Is my newborn covered?

Yes, automatically from birth, including for congenital and neonatal conditions, with no application required. This is the most important point for new parents.

What’s the difference between MediSave, MediShield Life and MediFund?

MediSave is your own CPF savings for approved medical uses. MediShield Life is insurance for large hospital bills. MediFund is a safety net for those who still can’t pay after the others. They work in sequence, not as alternatives.

Do I need an Integrated Shield Plan?

Only if you want private hospitals or higher ward classes. It wraps around MediShield Life instead of replacing it, MediSave covers part of the premium up to a limit with the rest in cash, and premiums rise steeply with age, so model that before committing.

Can I use MediSave for pregnancy?

Yes, for the delivery, plus an additional amount for pre-delivery expenses such as antenatal visits, scans and tests, which can be claimed retrospectively. Keep every receipt from the first appointment, since this is commonly missed. Either parent’s MediSave can be used.

Does MediSave cover braces?

No. Orthodontic treatment isn’t MediSave claimable, which surprises many families budgeting for it.

What to remember

MediSave is your savings, MediShield Life is the insurance, MediFund is the safety net. They stack.

Your baby is covered from birth automatically, including congenital conditions. No application.

Keep every antenatal receipt. The pre-delivery MediSave claim is routinely missed.

And check the current figures on the CPF and MOH sites, because they change.
